Применение β-блокаторов у детей в ОРИТ – результаты интерактивного опроса, краткий обзор и собственное мнение

Аннотация

Многие стационары используют β-адреноблокаторы у взрослых пациентов, но сфера применения у детей пока не определена.
Цель: получить сведения о применении β-блокаторов у детей в отделении реанимации и интенсивной терапии (ОРИТ).
Методы: анонимный опрос из 17 вопросов, касающийся использования β-блокаторов, был распространен через социальные сети, мессенджеры и электронную почту. Полученные ответы сопоставлены с данными литературы.
Результаты. Получены ответы от 48 респондентов. Опрос показал, что 66,7% из них используют β-блокаторы у детей, а 33,3% ‒ нет. Различия в применении β-блокаторов в общих и детских ОРИТ статистически незначимы. Большинство клиник (50%) применяют β-блокаторы у пациентов с врожденными пороками сердца, на втором месте ‒ сепсис (18,5%). Препаратом первой линии является эсмолол (34,4%), далее следует пропранолол (31,3%). Диапазон дозирования препаратов значительно различается (исключая эсмолол). Основной мониторинг и эхокардиография (40,6%) наиболее часто используются для контроля за применением β-блокаторов.
Заключение. Результаты опроса показали, что в России β-блокаторы у детей в ОРИТ используются очень ограниченно. Необходимо проведение крупных рандомизированных, многоцентровых, плацебо-контролируемых исследований, которые определят эффективность β-блокаторов у детей при различных клинических состояниях.
