Пациенты со злокачественными опухолями головного мозга, госпитализированные в отделение реанимации и интенсивной терапии: интенсивная терапия или паллиативный подход?

Аннотация
Благодаря развитию современных технологий за последние годы достигнут определенный прогресс в лечении злокачественных опухолей головного мозга (ЗОГМ). Однако прогноз для многих пациентов, страдающих ЗОГМ, остается неблагоприятным. Пациенты с ожидаемо неблагоприятным прогнозом требуют особого отношения и ухода, поскольку агрессивные терапевтические подходы не приводят к пол- ному или частичному восстановлению функций, и проводимое лечение не в состоянии продлить жизнь. В этом ключе госпитализация в отделение реанимации и интенсивной терапии (ОРИТ) при прогрессировании заболевания нередко может считаться нецелесообразной. При возникновении такой ситуации необходима консультация специалиста по паллиативной помощи и проведение консилиума с целью обсуждения возможностей и цели оказания медицинской помощи, тактики ведения пациентов в ОРИТ. Решающее значение для принятия решений имеет понимание прогноза и реалистичных целей лечения родственниками, а разумной альтернативой нахождения в ОРИТ могут стать оказание помощи в хосписе/отделении паллиативной помощи или патронаж на дому. Хотя преимущества такого подхода с точки зрения снижения нагрузки на ОРИТ очевидны, потенциальная роль паллиативной помощи в нейроонкологии недооценивается. Ранняя интеграция паллиативной помощи в систему оказания медицинской помощи больным со ЗОГМ не только обеспечит постоянное наблюдение за пациентами, улучшит качество их жизни, но и уменьшит число вызовов скорой медицинской помощи и число необоснованных госпитализаций в ОРИТ.
